With Some Small Paper Loss Near Edge, Partially Splitted Top & Bottom Edge, Light Stain on the Back Cover. With Number "5" Near the RIAA Logo on the Bottom Right Corner
Black LP Label with Circular Color Band. Does Not Have "Subsidiary of Capitol Industries" on Label.
