A superb example of a very rare 3 pronged "Phillips" pressing dating from August 1968, released on the Apple label: R 5722.
The "KT "purchase tax code appears in the run-outs on both sides, along with "Sold in UK" text.
Matrices: 7XCE 21185 -1 & 7XCE 21186 -1
Stamper codes: 5 KT 4 & 5 KT

Labels: Excellent 
Virtually no evidence of play clean and unmarked (please see photo's).
Visual: Excellent +
Both playing surfaces have retained much of their original sheen.   Some light hairlines visible only under a bright halogen light  (Please see photo's).

Both sides play flawlessly with no discernable issues or distractions. 
Cover Excellent 
Housed within a repro Apple Co sleeve which has only minor handling and creasing (please see photo's).
