Canadian Broadcasting Corporation Radio-Canada 12-inch 33 RPM radio transcription record No. 142 contains Glenn Gould's "String Quartet" played by the Montreal String Quartet. It comes with the insert shown in the third picture, which actually was intended to accompany a different recording of the same work by a different quartet.
The record looks to be in about VG+ condition, still nice and shiny, but with a few light surface scuffs and scratches, none of them too significant looking. I've played both sides on my Audio-Technica turntable, with no skips or jumps, and mostly very minimal background noise, but some occasional clicks or pops or ticking. Because of variations in tracking ability, filtering technology and needle weights, I can offer no guarantees as to how well it will play on your equipment. 
It comes with the custom cover shown in the pictures, which has partial splits at the seams and has been written on, front and back, with some of the writing whited out. It also has a plain white inner sleeve.
