 This extremely rare single/45 from WEIRD AL YANKOVIC is copyright 1981 and contains four demo & live tracks recorded before he landed his contract with Scotti Brothers:

1. "Another One Rides the Bus" LIVE from Dr. Demento Show

2. "Happy Birthday"

3. "Gotta Boogie"

4. "Mr. Frump in the Iron Lung"

UPDATE: I've received a couple of emails asking about the label—evidently some versions of this Placebo vanity release featured a blue label. This record was given by Al to the original owner: his girlfriend at the time of release. The labels on both sides are white. Scratched into the run-off portion is a handwritten Placebo "3626A" and "3626B" on either side.
