Vinyl Condition: VG+, few light scuffs and faint marks
Jacket Condition: VG+, light wear

I only use NM if I know for a fact the record has only been played a few times.
I ship all LPs with the record removed from the jacket in a sturdy mailer with cardboard reinforcement.
