Comes in plain white sleeve with album info sheet taped onto it, also comes with extra sheet.

"Promo/Test Not For Sale" printed on white labels. 

Only a few copies made, this is the only copy I have ever seen.

Super hard to find.

and this copy is in beautiful condition.
