This is a rare promo only picture disc. In 1981, (5) Ozzy Picture Discs were released to promote Diary of A Madman. Here is the Fourth of the set. Extremely Rare Low Production and its been said only about 100 exist. This is original and not some re-issue. 

OZZY OSBOURNE Live! (Super rare 1981 original US 3-track promotional - Only 12" picture disc EP including Mr. Crowley, You Said It All & Suicide Solution. This stunning example remarkably has managed to remain sticker sealed in the original custom stickered clear PVC sleeve from new, over 30 years ago! PD-9170).
