It is a test Acetate from Universal studios in Chicago from 1958, when Sam was building his  legacy with ground-breaking live performances, and equally good recordings. 
Both sides look and plays VG+ with some noise.  It is from the private collection of legendary Chicago DJ Richard Stamz, and  has his personal stamp across the label.

7” PACKAGING
We package our 45s in a thick cardboard 45 mailer. We ALWAYS take the 45 out of a picture
sleeve to avoid any seam splitting that could occur in shipment. We put the 45 and the picture sleeve,
where applicable, in a 45 plastic sleeve. We will NEVER ship your 45 loose.
We float the 45 between cardboard and 2 sheets of bubble wrap, and ensure there is no movement inside the box.
We use shipping tape on all spines to protect the integrity of the box, and to assure the safety of the contents.
All this is included in the low shipping price. We will clean your purchase
on our VPI machine if and when necessary.
This insures that you receive your item in the best possible condition.

 7” SHIPPING
We use the US Postal Service’s first class for shipping in the U.S. The buyer pays a shipping,
packing (see above) and handling fee of $3.00 for the first 7”/ 45rpm record and 
25c for each additional item in the same package. 
This includes USPS delivery confirmation (US only), box & packing material.

 Shipping to other countries is by the United States Postal Service's First Class Mail International. 
The rates for overseas is $12.50 for the first item, with an added $1.50 for each additional 45. 
Shipping to Canada is $8.50 for the first item, with an added $1.00 for each additional single.

 WE COMBINE ITEMS FOR THE BEST SHIPPING PRICE

 All items will be expertly packaged to prevent damage in shipment (see above).
