PRESTIGE RECORDS #7094 (DEEP GROOVE YELLOW/BLACK LABEL, BERGENFIELD ADDRESS - MONO). Cover is in VG++ condition (no splits at all, former owner's name on front and back cover in ink, light wear, few corner dings) and record is in VG+ to VG++ condition (few light scuffs/scratches, some pops & clicks but still very enjoyable listen - visual and play grading).
