Love "Forever Changes" LP. Elektra Butterfly label. 
Here's a great copy of this crucial LP from Love. I'm guessing this is a late 70s repress. It sounds really nice.

This record has been a well cared for member of my personal collection. Record is darn near flawless; jacket is a tiny bit worn and yellowed with age and some price tag residue is on the upper right corner.
