White label reads "TEST PRESSING", "DEMONSTRATION COPY" & "NOT FOR SALE". Vinyl plays EX/EX+ (looks NM with no scratches; just light surface noise in spots). Generic white cover with artist, title, and Test Press written with sharpie. Poly-lined inner sleeve.

For what it's worth: I was told that this belonged to a Portland, Oregon DJ who personally received it from Lou Reed. 

All (non-sealed) records are play-tested. Album(s) have been housed and will be shipped in a poly sleeve. Disc(s) will be shipped outside of their cover to avoid splitting. LP GRADING
M: Perfect. Sleeve, labels & inserts (if any) look like new without any marks. Vinyl looks unplayed.
NM: Nearly perfect. Sleeve does not have creases, writing, seam splits or any obvious defects. Labels & vinyl look clean without any obvious signs of wear. Vinyl plays almost perfect.
EX: Well cared for. Sleeve has slight signs of wear or light marks. Labels have light wear or marks. Vinyl may show slight scuffs or very light scratches and plays with some very minor audible noise.
VG: Obviously used. Sleeve has obvious wear which may include seam splits, stickers & writing (all will be noted). Labels show wear with possible stickers, glue residue or writing (all noted). Vinyl shows scuffs and/or light scratches. Upon playing, surface noise is evident but plays through without skipping.
G: Heavily used.
P  Abused.
