This is a 45 copy of Elvis Presley on Sun Record label.  You get Mystery Train on one side and I Forgot To Remember To Forget on the other.

This is a very nice copy.  It does have DH written in red ink on "I Forgot" side and a piece of white tape stuck to it.  I'm going to leave that alone and let new owner remove it. 

Looks to be in good condition with minor scuffs.  I do not have a record player so I do not know if it will play.  I just know it has very few marks.  I saw several of these going for $500 +.  I am starting it out at a low starting price with no reserve.
