Two-piece paper cover; I'll go with the explanation commonly found online, which is that this is a "controversial hand-numbered insert/sleeve which is quite widely regarded to be unofficial/counterfeit". I'd just note that, since there were very few of the Music-box record pressed in the first place (one source I saw said 500), it means that whoever printed the inserts had to get their hands on a large slice of the issue to accomplish their ruse. Whatever it is, and for what it's worth, this is #1 of 250.
